Hello, I have a patient who’s 1/2 of #8 tooth missing, and needs a crown. His Insurance company is refusing to pay it and I’m stumped as to why. I was wondering if anyone would be so gracious as to lend me a working narrative template they’d accept or an example of what has worked in the past for them? I’ve been working on this claim since March… anything helps!! Thanks.
