I need medical cross codes for: D0140 Limited oral evaluation – prob focused D0220 Intraoral – Periapical – First Film D047A Diagnostic wax up/Tooth D2962 Porcelain Veneer The patient was running and fell down a hill and onto her face and broke her upper incisor.
