Hi bethjones! J0588 is a valid codes for botox (as you found, J0858 is not). J0588 stands for: Injection, incobotulinumtoxinA, 1 unit The other 2 possible codes for botox type A are: J0585 – Botulinum toxin type A, per unit J0586 – Injection, Abobotulinumtoxina, 5 units Now – as for entering the NCD number – are you entering it into field 19, or onto the red shaded portion of the line item that the J code appears on? I have seen different insurers request that in either area, so it may be a good idea to include it on both areas just in case 🙂 Hope this helps!
