I am trying to bill botox injections for TMD I am using the code J0588 with the NDC on the vial but my claims are all rejected from the insurance stating NDC number. Procedure code of services rendered. I had previously tried billing J0858 but it denied as well. I need help ASAP with this issue. Thanks
