Hi Guest, It”s always a good idea to send a narrative along with the claim. Since Medical insurance doesn”t accept electronic attachments in box 19 on the claim form you can add a note stating “SOAP note mailed/faxed”. That way when they process the claim they will know additional documentation has been sent. For sleep appliances the HST is sent during the pre-authorization request. Hope this helps!
