D6104 D6011 D6040 D6066 D6057 D7210 Dr”s notes if that helps! patient presents to office for consultation concerning implant #9 patient has tooth #9 which was fractured after getting hit by a truck below the bone level a rct was completed and a post placed so Dr could place braces on the patient to pull the root down past the fracture and place permanent restoration . Dr confirmed that tooth is hopeless therefore new treatment plan will include extraction , bone graft, and implant placement Thanks in advance!
