D7295 – harvest of bone for use in autogenous grafting procedure 21210 – Graft, bone; nasal, maxillary or malar areas (includes obtaining graft) 21215 – Graft, bone; mandible (includes obtaining graft) The two codes above include obtaining the graft. Otherwise: 20900 – bone graft any donor area Hope this helps!
