ICD diagnosis Code

I have filed three times with Cigna for my patient”s dental implants. He had perio disease and had to have two teeth extracted and replaced with implants. Cigna keeps requesting an ICD diagnosis code. I looked up codes online and in a cross coding book and sent in. NOW Cigna says ”wrong codes”!!!! HELP???!!!!
